new patient information
step 1: Talk to us
Once you've filled out a request for services for either speech/language or occupational therapy, you will be contacted to further determine how we can assist your family and to schedule either a screen or an evaluation. If your child has a current evaluation (done within the past year), they will not need another evaluation prior to beginning therapy services at Sprocket.
At this time, we will also gather all required medical/insurance information.

(OPTIONAL) step 2: Screen
Screens are a supplementary service and are not a necessary part of the therapy process. A therapy screen allows a parent to make sure that our program is a good fit for their family before they begin the formal therapy process. During a screen, a Sprocket therapist will spend time with your child here at Sprocket and will make recommendations to you regarding how to progress with therapy based on their observations.
Screens are:
Screens are:
- 45 minutes long
- $100 (which will be credited towards your balance if you become a patient at Sprocket Therapy Solutions)
- informal
- not a necessary step in the therapy process
STEP 3: EVALUATION
A therapy evaluation is a formal procedure, and is required prior to beginning treatment sessions at Sprocket Therapy Solutions. An evaluation session lasts 1-1.5 hours and serves to determine where your child functions developmentally and how best to establish their plan of care. Therapists use a combination of standardized testing and skilled observation to generate a formal document detailing clinical findings for your review. At the end of this session session, the therapist will briefly share their observations with the child's guardians. It can take about 2 weeks for a therapist to complete writing up an evaluation document, in which they make recommendations regarding frequency of therapy visits and duration of the plan of care. The therapist will also work with families and caregivers to set several goals for the child, to be listed at the end of the evaluation document, based on the specific needs of the family and the findings of the evaluation session.
STEP 4: follow up treatment SESSIONS
Each treatment session is 1 hour long and most clients attend therapy 1x per week. Prior to your evaluation, you will be assigned a standing therapy appointment time, which will repeat every week. Parents are encouraged to participate in therapy sessions as necessary, but we also reserve the right to politely ask parents to refrain from sitting in on therapy sessions if it disrupts the child's performance. We treat patients Monday - Thursdays from 8am - 6 pm.
